Process Change Documentation

I wanted to reach out to see if anyone had any effective ways to make sure process changes were getting documented correctly. Currently we use a paper form and any time you make a process adjustment you log it onto the form with the Date, Time, and Reason for Change. Too often, people are working on multiple things and often forget to document properly after making a change. We are looking to improve our process by adding some accountability and potentially be able to track who is making undocumented changes. Current potential options include individual passwords or keys for anyone who makes process changes, as well as a reminder prompt from the press that states "was this process documented?" I was hoping someone from the MAPP community would have something more beneficial than just hoping people get better at documenting.
